Notice Regarding Health Information
In some of your interactions with us, we may collect medical or health information from you on behalf of a healthcare provider that provides services to you that may be subject to specific protections under applicable state laws. This Privacy Notice does not apply to Protected Information. When we collect Protected Information, we do so as a service provider on behalf of the healthcare providers who provide healthcare services to you, and we will only use and disclose Protected Information in accordance with applicable laws and our agreements with those healthcare providers. Sources of Personal Information
